However, we believe that in order for you to take active control of the health and wellness for you and your family, it is essential to work with a primary doctor to get the regular check-ups and preventative care you need and have that trusted healthcare professional help you navigate the complicated healthcare system. That is why the third part of our program was developed – the C&K Medical Home Program. In addition to a great place to work, most of our stores are located in small towns that offer scenic backdrops, family friendly communities and less hustle and bustle than you might find in the mega-cities many companies call home. A career with C&K also provides an excellent benefit package, which includes: What is a Medical Home?A Medical Home is a doctor or group of primary care providers who agree to provide C&K employees and their participating family members: Same-day appointments for urgent issues; Coordination of your care with specialists; and Encouragement for preventative care and wellness. How do I participate in Medical Home?
If you are enrolled in any of our C&K Health Plan options, all you need to do is fill out the Medical Home Designation form with a participating Medical Home provider and return it to the Benefits Department. Once C&K’s Benefits Department receives the form, your Medical Home designation will be effective the first day of the following month.
What benefits do I get for participating in Medical Home?In addition to same-day appointments for you and your family members for urgent issues, coordination of care, and a wellness coach, the following copays and limits for preventative services are reduced:
Please Note:Some restrictions and limitations do apply to these benefits. Also the Plan reimburses 100% of charges when you see a provider in network and 100% of usual and customary charges (UCR) of charges when a provider is not in network. Your Medical Home provider may or may not be in network. If your Medical Home provider is not in network and their charges exceed UCR, you may be responsible for the difference. Please refer to the Benefit Schedules in the C&K Market Health Plan for all other benefits provided. Can I designate a Medical Home and my other family member designate a different Medical Home?Yes. How much does it cost to designate a Medical Home?Designating a Medical Home provider costs you nothing – just sign-up with a participating Medical Home and you receive the same-day appointments, specialist coordination and wellness assistance at no charge to you. Additional fees may apply for other services received. What happens if I want to change Medical Home providers?Complete new Medical Designation Form and send to Human Resources. Human Resources will notify your providers directly if you have changed designation. Do I have to change my doctor?No. If you are seeing a doctor who chooses not to participate in the Medical Home Program, you will only have additional copays. You still receive wellness benefits according to the C&K Health Plan. How can I find a Medical Home provider?Review the pdfs of C&K Medical Home providers above.
